The Spreadsheet is made up of two worksheets.These are The Respondent Information Worksheet, where the data from the top of the survey is entered.; and the Survey Results Worksheet, where the results of the questions 1-24 are entered. You can move between the worksheets by clicking on the tabs at the bottom of the screen. For example: This is Louise who is 23 and has lived in the community for 19 years. Her record looks like this: M F <18 19-25 26-35 46-55 56-65 >65 YRS 1 1 19 You can enter the data from the survey forms in two ways: We recommend the SECOND Option if you feel confident using EXCEL 1. You can use the spreadsheet as a table and adding information about each person along the rows OR 2.: You can complete the first record along the first row beneath the headings, put your cursor in any cell in this row and go to Data on the menu bar and click on Form in the drop down menu. This will bring up a database form, which corresponds with all the headings in the spreadsheet. This is a much easier way to enter the data. As you enter the results of each survey you will need to switch between the 2 Worksheets to record the results.
